PET GROOMING RELEASE OF LIABILITY & HOLD HARMLESS AGREEMENT
Please read carefully before your first appointment. Submission of this form confirms your understanding and agreement to the following terms.
I certify that I am the legal owner or authorized agent of the pet(s) listed on this form and authorize grooming services to be performed.
I understand that grooming procedures involve the use of sharp tools, clippers, scissors, dryers, bathing equipment, and handling of animals that may move unexpectedly. While every reasonable precaution will be taken, grooming carries inherent risks.
I certify that my pet is healthy enough for grooming services and I have disclosed any medical conditions, allergies, injuries, sensitivities, behavioral concerns, anxiety, aggression, bite history, or other relevant issues.
I confirm that my pet is current on legally required vaccinations and is free from contagious illness, fleas, ticks, and parasites. If parasites are discovered, additional cleaning or treatment fees may apply.
If my pet's coat is matted, tangled, impacted, or poorly maintained, I understand that de-matting or shaving may be necessary for humane and safe grooming. I understand that pre-existing coat neglect may reveal or contribute to skin irritation, redness, cuts, abrasions, bruising, itchiness, or uneven coat appearances.
For senior pets or pets with medical concerns, I understand there may be increased risk due to age, stress, or health condition. Grooming services may be shortened, modified, or discontinued if necessary for the pet's safety and wellbeing.
In the event of illness, injury, or emergency, I authorize the groomer to seek veterinary treatment if I cannot be reached. I agree to be financially responsible for veterinary costs unless caused solely by gross negligence.
I understand that requested haircut styles may be modified based on coat condition, humane treatment standards, pet tolerance, behavior, and time required.
I voluntarily assume the normal risks associated with pet grooming services and release, indemnify, and hold harmless the groomer, salon owner, employees, contractors, and agents from claims, damages, losses, costs, or expenses arising from grooming services, except in cases of proven gross negligence or willful misconduct.
I understand payment is due at the time of service and cancellation, late arrival, or no-show fees may apply.
By checking the agreement box and providing my electronic signature, I acknowledge that I have read, understand, and agree to all terms above.
